Your Eye Exam
If you are visiting our practice for the first time, we will need you to bring your insurance card, picture ID and completed form (please see form section below). Kindly arrive at least 20 to 30 minutes before your appointment to ensure there is enough time to present all relevant information at reception.
Patient Form
To ensure that we have all your relevant contact and medical information, we kindly request that you download and fill in the below form to bring with you to your next appointment. Rest assured, we will not share your information with third parties – this information is exclusively for our records.
Patients Portal
Our patient portal allows you to quickly access services on our website and to get access to previous documentation. Simply create your profile to get access to various benefits such as pre-booking appointments, making payments and referencing previous payments for your records or tax purposes.
Financing
We recommend that all patients contact their medical insurance providers prior to appointments to establish cover and whether our practice will include coverage. If you need any assistance with this, you are welcome to contact our reception. Kindly note, we advise that cover is determined at least one week before visiting the Holtebeck Eye Center.
Order Contacts
Do you need a refill for your contact lens prescription? Simply sign up on our patient portal, if you haven’t done so yet, and then order your contact lenses which we will, upon payment, either deliver to your door or arrange to be packed and ready for pickup when you visit our practice.
